To become a primary care client, you can pick up a New Client Application Form from reception at our Belleville or Trenton offices or print one off directly from the link below and follow the instructions on the form:
BQWCHC continues to receive a high number of new client applications. While the demand for our services grows, it has resulted in the need for a wait list. Your application will be added to our waitlist, and you will hear from us as soon as we are able to place you with a health care provider.
Who We Serve
BQWCHC serves those who reside in the Belleville and Quinte West area. Special consideration is given to those who experience multiple barriers to accessing care (for example, those who are racialized, 2SLGBTQI+, those struggling with mental health or addictions, and those living on a fixed or limited income) AND without primary care.
Programs & Workshops
BQWCHC offers many group programs and services to improve your health and wellbeing. Most of our programs are free and open to anyone (not limited to those rostered to primary care at the centre).
