As a client at BQWCHC , you have rights as well as responsibilities.

You have the right to:

  • Be treated with dignity and respect
  • Access services in a safe and secure environment
  • Have your privacy protected and your information kept confidential
  • Make decisions about how your personal health information is used
  • Review your health file
  • Make decisions about your own care, including decisions to accept, decline or discontinue care
  • Information to inform decision-making about your care
  • Receive quality care
  • Provide feedback or issue a complaint by filling out this form.
  • Participate in shaping the work and direction of BQWCHC

As a client, you have a responsibility to:

  • Treat others – staff, clients, students and volunteers - with dignity and respect
  • Respect the centre's values, policies, and property
  • Be informed and involved in making decisions about your care to the best of your ability
  • Participate in your own care and follow care plans to the best of your ability
  • Advise BQWCHC of any changes in personal information
  • Advise BQWCHC when you are unable to keep an appointment.
