You have the right to:
- Be treated with dignity and respect
- Access services in a safe and secure environment
- Have your privacy protected and your information kept confidential
- Make decisions about how your personal health information is used
- Review your health file
- Make decisions about your own care, including decisions to accept, decline or discontinue care
- Information to inform decision-making about your care
- Receive quality care
- Provide feedback or issue a complaint by filling out this form.
- Participate in shaping the work and direction of BQWCHC
As a client, you have a responsibility to:
- Treat others – staff, clients, students and volunteers - with dignity and respect
- Respect the centre's values, policies, and property
- Be informed and involved in making decisions about your care to the best of your ability
- Participate in your own care and follow care plans to the best of your ability
- Advise BQWCHC of any changes in personal information
- Advise BQWCHC when you are unable to keep an appointment.
